As teaser season for Path of Exile 2’s upcoming 0.5.0 update, called Return of the Ancients, kicks into full gear, publisher and developer Grinding Gear Games might be planning some changes to the action-RPG’s Delirium endgame mechanic.
A short trailer shared on April 30 appears to be heavily drenched in Delirium’s aesthetic. Coated in a thick, dark atmosphere, it shows a figure who appears to be Elder Madox – a character we encounter during the campaign – descending from the mountain we find him on to “stop this insanity.”

The spirit owl accompanying him flies into a tree whose side is covered in the grey mist that’s all too familiar to anyone who’s engaged with PoE2’s Delirium mechanic. Doing so reveals the figure of a child trapped inside a crystal, who, in a terrified voice, asks to be saved.
The unsettling tone of the music and colors used in Path of Exile 2’s latest teaser trailer could be developer Grinding Gear Games’ way of hinting that the Return of the Ancients league will bring a major rework to the Delirium endgame mechanic.
At the same time, we could be looking at content meant for a new campaign act, an equally exciting prospect.
The developer will fully reveal what’s coming in PoE 2’s 0.5.0 update on May 7, 2026, so we won’t have too long to wait until we know for sure if Delirium is, indeed, getting reworked.
The update is then slated to go live on May 29, 2026, bringing with it the long-awaited endgame rework that was initially planned for its predecessor.
Given how it launches five months after the prior update and took longer to develop, rather than the usual four, expectations for Path of Exile’s 0.5.0 update are rather high.
It’s currently unclear where that leaves the content schedule for the remainder of the year and whether or not the action-RPG’s 1.0 launch is still slated for 2026.
